Billing, Payment and Warranty

Billing and Payment:

We will send you a bill at the end of each month. The term is net 25 days. Any balance not paid 25 days after statement date will be charged to the Doctor's credit/debit card on file. Please print and fill out the form below and include it in your next case to us or you can fax the form to us at 808-955-8883. As a safety measure we do not keep your credit/debit card information in our computer but at Intuit Quickbooks Pro off site and encrypted.

Credit and Remake Policy:

All dental appliances are custom made and cannot be refunded. However, a credit will be issued and applied to your account for any cases with problems pertaining to our lab work. To receive credit for a case, the original case must be returned along with the models, within 30 days from the invoice date. Case returned after 30 days can be remade at no charge.
Any remakes pertaining to our work are done free of charge.
Cases must include the original dental restoration along with the models

Warranty of Work:

We guarantee our work for 5 years of clinical use, except for the following:
Immediate partials, dentures, and acrylic partials
Orthodontic appliances
Temporaries or Veneers
Doctor’s design modification of appliance, adjusting, altering or change of material, or shade change
Cases completed after notification to the doctor of poor impression or model
Note: Warranty does not cover natural wear and tear associated with prolonged use and/or poor dental hygiene and maintenance.
